Visually, very little.
Legos have slightly more rounded tops, but that's about it,
From what I have seen just looking at them.

The only real difference is Legos say legos on every piece and these say mega bloks on them 8)

Mega bloks may work with mega blok only sets, but that is just a guess of mine!
