Since our seller hasn't answered yet, Yes you have to have a subscription to Sirius for this to work. And yes you can use headphones assuming the audio out jack is in good working order. I have one of these and love it. However the flat rate shipping quote he posted as $8.50 is binding on Listia, meaning it will be $8.50 to ship anywhere in the US. Therefore "shipping rates may vary" should only apply to outside the US.
